A

ACT – Assertive Community Treatment-Intensive, 24/7 services for people with serious mental illness

AD – Advanced Directives – Allow persons to determine in advance the kind of medical care they prefer. Includes care power of attorney, “do not resuscitate” (DNR) orders, and other types of legal documents.

AMRTC – Anoka-Metro Regional Treatment Center

ARMHS – Adult Rehabilitative Mental Health Services

BCBSM – Blue Cross/Blue Shield of Minnesota

C

CADI – Community Alternatives for Disabled Individuals – CADI is a home and community care program that pays for health care services in the home of an individual who requires the level of care of a nursing facility. This is a medical assistance program approved by the Centers for Medicare and Medicaid Services, formerly the federal HCFA.

CAP – Community Action Programs, which may also be identified as CACs or Community Action Councils.

Case Management/Care Coordination – The arrangement, coordination, and monitoring of services to meet the needs of clients and their families. Case management activities include, but are not limited to: individual service plan development and monitoring; developing, securing, and coordinating services; monitoring and evaluating client progress; and ensuring that clients’ rights are protected.

CBHH – Community Behavioral Health Hospitals -16-bed inpatient acute psychiatric hospital operated by State Operated Services.

CD – Chemical Dependency

CD/MH – Chemical Dependency/Mental Health

CMS – Centers for Medicaid and Medicare Services. This is the federal agency responsible for administering Medicare, Medicaid, CHIP (Children’s Health Insurance), HIPAA (Health Insurance Portability and Accountability Act), and several other health-related programs.

C.O.A.S.T. – Community Outreach And Stabilization Unit, the Mental Health Unit of the Saint Paul Police Department. The COAST Unit consists of the Mental Health Resource Team (MHRT), the Recovery Access Program (RAP) and the Police Homeless Outreach Program (PHOP).

CTSS – Children’s Therapeutic Services and Supports

D

DA – Diagnostic Assessment

DD – Developmental Disability – Diagnosis of a developmental disabilty or a related condition resulting in substantial functional limitations or deficits in adaptive behavior and manifestation these conditions before the 21st birthday of person with a this diagnosis

Detox – Applies to receiving centers required by law to be established in every mental health area in the state in order to detoxify individuals found to be publicly intoxicated or high.

DHS – Minnesota Department of Human Services

DOB – Date of Birth

DSM-IV Diagnostic and Statistical Manual of Mental Disorders, 4th Edition-Revised – American Psychiatric Association’s official manual of mental disorders. Manual contains glossary of descriptions of the diagnostic categories.

EA – Emergency Assistance

EAP – Employee Assistance Program – Services designed to assist employees, their family members and employers in finding solutions for workplace and personal problems.

EBD – Emotional and Behavioral Disorders

EHR – Electronic Health Record

EMR – Electronic Medical Record

ER – Emergency Room

FBS – Family-Based Services. Services that are provided to a family in their own home with a goal of preserving the family unit and preventing out-of-home placements.

G

GA-EA – General Assistance-Emergency Assistance (also known as EGA) – Monies provided to eligible persons in crisis situations which receiving EGA will resolve, provided that the individual is unable to resolve the crisis situation on their own without EGA and that the situation will result in severe hardship for the individual if not resolved. Must be a bonafide emergency.

GRH – Group Residential Housing – GRH is a state-funded income supplement program that pays for room and board costs for low-income persons who have been placed in a licensed or registered setting with which a county human service agency has negotiated a monthly rate.

HIPAA – Health Insurance Portability and Accountability Act

HMO – Health Maintenance Organization – An organization that provides an agreedupon set of basic and supplemental health maintenance and treatment services to an enrolled group of persons within a particular geographic area. The HMO is reimbursed for those services through a predetermined, fixed periodic prepayment made by or on behalf of each person or family unit enrolled in the HMO without regard to the amounts of actual services provided

Hold Order – This is a term used when it is necessary for a police officer or doctor to put an emergency hold on an adult or child in order to protect that person from either themselves or someone else. A hold order gives officials the right to place a person in a facility where they will be protected. Hold orders are effective for 72 hours excluding weekends and holidays.

HRA – Health Risk Assessment.

I

ICF – Intermediate Care Facility – Term is obsolete. Formerly used to identify a facility certified by the state Department of Health to provide, on a regular basis, health-related services to individuals who do not require hospital or skilled nursing facility care, but whose mental or physical condition required above the level of room and board.

ICU – Intensive Care Unit

IMD – Institution for Mental Diseases – Classification under Medical Assistance (MA) that denotes a hospital, nursing facility or other institution of more than 16 beds that is primarily engaged in providing diagnosis, treatment or care of persons with mental diseases.

IMR – Illness Management and Recovery – an evidence-based mental health practice

IRTC – Intensive Residential Treatment Services – Facilities with up to 16 beds that provide short-term, intensive 24/7 mental health treatment.

LAC – Local Advisory Council – county mental health advisory council

LADC – Licensed Alcohol and Drug Counselor

LCP – Licensed Clinical Psychologist

LICSW – Licensed Independent Clinical Social Worker

LMFT – Licensed Marriage and Family Therapist

LOCUS – Level of Care Utilization System – is a level of care instrument developed by the American Association of Community Psychiatrists. For adult MH-TCM services, it will be used for the purpose of helping to determine appropriate referrals to services.

LOS – Length of Stay

LP – Licensed Psychologist

LPN – Licensed Practical Nurse

LSW – Licensed Social Worker

LTC – Long Term Care – Medical and social services and supports needed by
individuals who, through age or infirmity, need assistance to remain independent.

M

MA – Medical Assistance – also known as Medicaid or Title XIX of the Social Security Act

MAT – Medication-Assisted Treatment (MAT) – the use of medications, in combination with counseling and behavioral therapies, to provide a “whole-patient” approach to the treatment of substance use disorders

MACMH – Minnesota Association for Children’s Mental Health

MACMHP – Minnesota Association of Community Mental Health Programs

MACSSA – Minnesota Association of County Social Service Administrators – formerly referred to as County Directors Association.

MCO – Managed Care Organization

MCT – Mobile Crisis Team

MDE – Minnesota Department of Education

MDH – Minnesota Department of Health

Medicare – Medical coverage through Social Security.

Medicaid – The Federal Medicaid program is called Medical Assistance in Minnesota.

MH – Mental health.

MFIP – Minnesota Family Investment Program – A program designed to simplify the structure and administration of the public assistance system in Minnesota. The focus is on using existing resources more effectively and efficiently and includes consolidating Temporary Assistance to Families (TANF), Family General Assistance (FGA), and food support (formerly known as food stamps) and to help promote recipients’ transition to self-sufficiency

MHCP – Minnesota Health Care Programs – PMAP/PGAMC (Prepaid Medical Assistance Program/Prepaid General Assistance Medical Care Program), MinnesotaCare, Minnesota Senior Health Option, Minnesota Disability Health Option, Minnesota Senior Care, and Minnesota Senior Care Plus, other

MI/CD – Mental Illness/Chemical Dependency

MinnesotaCare – This is a Minnesota program that makes health care more affordable. The monthly premium is based on income and family size; applicant cannot be eligible for Medical Assistance.

NAMI-Minnesota – National Alliance on Mental Illness – Minnesota Chapter

NP – Nurse Practitioner

OCD – Obsessive Compulsive Disorder

PCA – Personal Care Assistant

PCP – Primary Care Physician – A physician whose practice is devoted primary to internal medicine, family/general practice and pediatrics (or primary care provider)

PMAP – Prepaid Medical Assistance Program. Managed health care for MA recipients in non-CBP counties. Services are provided by HMOs.

Prior Authorization – Authorization from Medical Assistance or an insurance company prior to a client being admitted to a hospital or treatment facility to guarantee that they agree the treatment is appropriate and that the medical costs incurred by that client will be paid.

PTSD – Post-Traumatic Stress Disorder

Residential Facility – Refers to an out-of-home placement facility for children or adults. This does not include foster care.

RFP – Request for Proposal – A formal notification that funds are available and invitation to service providers to submit grant proposals.

RTC – Regional Treatment Center – A state-operated institutional facility providing 24- hour a day care and treatment for persons diagnosed as mentally ill or chemically dependent. Formerly known as state hospitals.

Rule – Rules are either the interpretation of laws or the issuance of regulations by agencies that have supervisory authority over our activity. The primary rules we are concerned with are those issued by the Department of Human Services. Rules number well over 100; a compilation of these rules can be found in the office “library.” Frequently quoted rules include: Rule 49, the rule that sets the rate reimbursements system for nursing homes; Rule 52, this establishes the rate reimbursements system for ICF-MR facilities; and Rules 34, 35, and 36, these apply to licensing standards for facilities serving people with developmental disabilities, chemical dependency, and mental illness, respectively. Rules have the force and effect of law.

SBIRT – Screening/Brief Intervention/Referral for Treatment

SED – Serious Emotional Disturbance

SMRT – State Medical Review Team – determines disability status using Social Security Administration criteria

SMI – Serious Mental Illness

SNAP – Supplemental Nutrition Assistance Program (name of food support program). The ‘S’ emphasizes that the program offers benefits are designed to help supplement clients’ monthly food resources, while the ‘N’ stands for providing a nutritional benefit.

SNBC – Special Needs BasicCare – A voluntary managed care program that combines Medicare and Medicaid financing and services for people ages 18 through 64, who have a certified disability, and are eligible for Medical Assistance. People who enroll continue to receive most home and community-based services Medical Assistance fee-for-service.

SPMI – Serious and Persistent Mental Illness

SW – Social Worker

TBI – Traumatic Brain Injury

TCM – Targeted Case Management

TEFRA – Tax Equity and Fiscal Responsibility Act of 1962 – Federal Public Law 97-248. This term is also commonly used to describe the optional medical assistance (MA) category of eligibility enacted under TEFRA Subs. 134 for disabled children who require a level of care of an institutional setting and who, if they lived in an institution, would be MA eligible but for whom care at home is less expensive for the MA program.

UCARE – Minnesota UCARE

UR – Utilization Review – A formal assessment of the medical necessity, efficiency and/or appropriateness of health care services and treatment plans on a prospective, concurrent or retrospective basis.

V

VA – Veterans Administration or Vulnerable Adult

VA/DD – TCM – Vulnerable Adult/Developmental Disabilities – Targeted Case Management. Services are provided on behalf of a vulnerable adult or person with developmental disabilities. These are services intended to coordinate and link social and other services, designed to help gain access to needed protective services, social, health care, mental health, habilitative, educational, vocational, recreational, advocacy, legal, chemical, health and other related services.

Vulnerable Adult – A vulnerable adult is any person, 18 years of age of older, who is: 1) A resident or inpatient of a healthcare facility or receives services from a licensed home care provider or a person or organization that provides personal care assistance under the state’s medical assistance program. 2) An individual who lives at home or in a facility and is unable or unlikely to report maltreatment of him or herself because of a physical or mental infirmity or other physical, mental or emotional dysfunction that impairs a person’s ability to provide adequately for his or her own care without assistance.

W

WIC – Women, Infants and Children Program – Supplemental food program for nutritionally at-risk pregnant or nursing women, infants and children to age 6.
